1. Application letter address to Chairman IRS.
2. Photocopies of Memorandum and Articles of Association, (Original to be presented for sighting).
3. Photocopies of certificates of Incorporation, (Original to be presented for sighting).
4. Banker’s reference on the company or two Directors with regards to lotto operations which requires payment of N50 million licensing fee/optimal N15m annually for 5years
5. Letter of commitment to pay the licensing fee as required by the State. Current fee is N50 million naira only/ optimal N15m annually for 5years.
6. Completion of Promoter’s Licence Form STB 94A at N100,000.00
7. Licensing fee of Fifty Million Naira Only/ optimal N15m annually for 5years.
(N50,000,000.00) on or before the date of submission of the duly filled promoter’s licence Form STB 94A.
8. Income Tax Clearance Certificate N250,000.00(Two Hundred and Fifty Thousand Naira only for three (3) years per Directors.
NOTE: THE GOVERNOR IS THE ONLY AUTHORITY
FOR THE APPROVAL OF A LOTTO LICENCE IN THE STATE.
